A State-Approved Program Verification — Teacher, or a Proof of Program Enrollment (for teachers only) must be submitted or on file with Teacher Certification except for those teachers applying under the Professional Out-of-State. This form will be valid for the following situations: If a university teacher preparation program or a state-approved (non-university) program was completed, submit the complete State-Approved Program Verification.
